Don’t have a clue worth and only one I’ve ever seen

I’ve tried time and time again to find more about this booklet but I’ve seen two pictures on google and a PDF on a history website of the book that’s all and it’s also signed by a name I can’t make out. Any help or possible knowledge would be great! Thanks y’all.
