This piece is a Capo di Monte piece from Azzolin Brothers in Italy and purchased in the Azzolin Brothers store near Chicago.

This one is a lot better quality than a lot of Capodimonte examples, but they still are a bit of a hard sell these days. Here's an auction link for you for the last year:
